Adoption Application

If you wish to adopt, we require a two-week provisional period.  This allows the potential adopter to determine if this dog will work for them. After the two weeks, the potential adopter must adopt, foster, or surrender the dog back to Canine Friends Foundation.

At anytime after the adoption, Canine Friends Foundation will take the dog back regardless of the reason.

Describe your yard. Does it have a fence, kennel run, or other? If fenced what is the height of the fence, type of fence, and approximate size of the fenced in area? Please also include the number of gates and if these gates have security locks or latches.
Please list the vet office you currently use, if you have one.
Do you have animals?
This means sit, stay, down, place etc... We offer to teach fosters to train dogs for free. Explain your background please.

This dog will be returned to Canine Friends Foundation if it is no longer allowed in the adopter's home. You must agree to this in order to foster or adopt.
